Brand design to help a health tech company share its vision for improving patient outcomes.

Home Health Notify (HHN) is a real-time communication app that enhances care coordination with its HIPAA-compliant platform, helping home health agencies improve efficiency and reduce hospitalizations. To support its growth, HHN partnered with the Work Department to refine its brand identity and develop strategic digital content.

The company already had a logo and select brand assets in use that it was happy with. Building off of these assets, we refined the brand by formalizing a color palette, font, and graphic style that we packaged into a brand and style guide. This helped the company to establish a cohesive voice and visual identity, reinforcing HHN’s position as trustworthy, collaborative, and patient-centered.

Additionally, we copyedited and designed an ebook, Home: The New Epicenter of Healthcare, using storytelling and custom graphics to highlight industry shifts and HHN’s role in improving patient outcomes. Written in 2022, this thought leadership piece explained shifts in healthcare policy, reimbursement models, and patient care expectations. It positions Home Health Notify as a key solution to the challenges facing home health agencies, particularly in preventing unnecessary hospitalizations and improving coordination across care teams.

These marketing tools have helped HHN grow by strengthening its credibility, clearly articulating its role in the healthcare ecosystem, and providing valuable insights to potential clients.
